as per Constitution approved at the EHMTIC 2012 in London 22nd September 2012.

Being a member of IFHN is free of charge. The member carries out own expenses in connection with headache conferences and local IFHN meetings.

By submission of this information, I hereby confirm applying for membership of the International Forum for Headache Nurses (IFHN) as per the constitution of the Forum.

I permit the Forum to:
– hold this information in an electronic database
– publish it in a membership register
– use it for communication within the Forum
– make it available for use by all other members
